Total knee Arthroplasty: TKA
TKA stands for Total Knee Arthroplasty. It is a surgery to replace the weight bearing surfaces of the knee joint to eliminate pain and lack of motion in the knee. Arthroplasty means “surgical repair of joint.” It is an orthopedic surgical procedure where the articular surface (location where bones connect) of a join is replaced or realigned or remodeled. It is typically done to restore knee function after damage. Damage can be caused by arthritis or some other type of trauma to the area.
This is a group of mechanical abnormalities involving the degradation of joints including articular cartilage (connective tissue) and subchondral (rounded end) bone. Symptoms can include pain in the joint, stiffness, tenderness and locking of the joint. This can be caused by your heredity, or your body’s deficits (developmental, metabolic, and/or mechanical). When bone surfaces become less well protected by cartilage, the bone may be exposed and damaged. This can result in decreased movement; muscles in the area can atrophy and ligaments can become more lax.
Arthritic and non-arthritic joints may develop “arthrofibrosis.” In this condition the knee fails to straighten fully. This is called a flexion contracture. Persons with this condition walk with the knee bent to varying degrees. The angle that the knee makes can be measured and is labeled as the degrees of the flexion contracture. The higher the number, the more bent the knee is.
What is the X10?
The X10 is a knee rehabilitation machine. It gently and quickly helps knee replacement patients regain their knee’s range of motion and strength. It can be used prior to surgery as a “prefab” tool but is most commonly used in the weeks immediately after surgery in the home. Use of the X10 dramatically speeds up recovery from knee replacement by many months with dramatically less pain than with traditional physical therapy.
Is X10 covered by insurance?
In Michigan X10 is available in your home at no additional cost to you by Michigan Community VNA as well as other home care companies. As long as your insurance covers in-home rehabilitation, X10 can be part of your care for the first 2-3 weeks post-surgery, while you are still home bound.
X10 is also covered by Blue Cross Blue Shield of Michigan for use in your home with or without home therapy. No matter where you are for your recovery, if you have BCBS MI, you are covered.
In an outpatient setting the X10 can be included in your rehabilitation in any one of the clinics which currently have the X10.

What can you tell me about delivery and setup for the X10 in my home?
The X10 is delivered a few days before you go to the hospital for your surgery. We set up the machine in a convenient place (preferably near your TV as you can enjoy entertainment while using the machine). It takes up as much room as does an easy chair (approximately 3.5 feet x 3.5 feet). The X10 plugs directly into your wall power outlet with a normal three-pronged plug. It is easy to get on and off the machine, the adjustable chair will be set-up for you during the first visit. You will be instructed on how to use the X10, and within a few minutes you can begin your therapy. At the end of your weeks of therapy we will pick up the machine from your home.
Is X10 a CPM machine?
In a word, no. The X10 is the first in a new class of far superior machines when compared to the CPM. These machines are called PMKR machines, PMKR stands for Pressure Modulated Knee Rehabilitation.
CPM (continuous passive motion) machines are based on technology from the 1970’s that simply a patient’s leg between two set points without helping to expand the range of motion in any meaningful way.
PMKR machines are far more effective for knee patients. The machines read a patients knee pressure and adjust to the level of tension to minimize discomfort and deliver effective therapy. X10 technology is based upon two unique method patents that allow us to move the knee joint in a fundamentally different way that leads to rapid gains in flexibility and strength without creating pain. This helps the patient work with the therapy and overcomes the chronic therapist issue of Protective Muscle Guarding. X10 incorporates active and passive ROM, along with isotonic and eccentric strengthening.

Why is X10 better than other forms of rehab?
X10 is more gentle than traditional physical therapy. You get more therapy in your home than you do with traditional home care. Can X10 help me even if I am long past surgery?
Possibly. We have had patients use the X10 long after surgery with success. There is not yet clinical data that supports using X10 more than two months post surgery, however patients have seen significant gains in range of motion and strength all the same well after their surgery.
Can the X10 help me recover after a Manipulation
Yes, yes, yes. The X10 is highly effective after a MUA (Manipulation Under Anesthesia). It is of paramount importance that you begin working on the X10 within 24 hours of your Manipulation. Scar tissue is often the culprit for patients who require a MUA. The X10 can help you beat the race against the formation of scar tissue, but research has shown that it matters how soon after surgery you start on the machine. The sooner the better.
What if my surgeon does not know about X10?
X10 is relatively new. While many thousands of people have recovered from knee surgery using the X10 since early 2013, many physicians have simply not yet heard of X10, and others are slow to adopt new technology. We have a great deal of information for physicians that demonstrates the effectiveness of the X10 vs. current standard of care and illustrates the difference it can make for their patients. Do I have to follow my hospital’s home care program?
No. The choice of how to rehabilitate from a knee surgery is up to you. Your surgeon and hospital discharge planner may recommend specific home care solution that is a part of their medical system. At this time Michigan Community VNA is the only home care agency in Michigan with the X10™.
Can I use my own outpatient facility after using the X10 in my home?
The choice of outpatient therapy provider is up to you. You can choose any physical therapy clinic you like. You can also choose to have X10 in your home for your post surgery therapy.
Currently the X10 is available in a number of physical therapy clinics across SE Michigan. We are adding to this list on an ongoing basis. We will be expanding to other facilities and states very soon (updated Winter 2016).
Do I need a prescription for X10?
Yes (& no). You don’t need a separate prescription for X10 Therapy. Many patients decide to add the X10 to whatever therapy solution they have arranged. If they are not covered patients can pay for X10 on their own. We offer financing with very reasonable terms through our partner, Care Credit.
If you live in SouthEast Michigan you can get a prescription (or “orders”) from your surgeon for home care services (and select a home care provide that has X10) or for specifically for the X10 if you have Blue Cross Blue Shield of Michigan. What is the difference between Inpatient Care, Home Care, Outpatient Care and a Skilled Nursing Facility?
Inpatient Care is provided by a facility that can care for you around the clock. You are transported from the hospital to a skilled nursing facility where you stay and receive 24/7 nursing case and physical therapy.
Home Care is in-home rehabilitation and nursing care provided by therapists and nurses that comes to your home 3-5 times per week while you are home bound.
Outpatient Care is provided in an outpatient physical therapy clinic. You travel on your own to these facilities where you work with a physical therapist on your rehabilitation. This is usually covered by your insurance for a finite number of visits after your knee replacement.
A Skilled Nursing Facility provides a type of residential care. It is a place for people who require continual nursing care and have a significant difficulty coping with the required Activities of Daily Living. Nursing aides and skilled nurses are usually available 24 hours a day.
